The player's date of birth
The player's team and association in 2009-2010
The player's division (ex: Novice, Pee-Wee, Atom) in 2009-2010
The player's class (ex: AA, B, CC) in 2009-2010
The player's preferred position (ex: forward, defence, goaltender)
The player's preferred group (ex: A, B, C+, C)
The program(s) for which the player is being registered (ex: Hockey School, Pre-Season Camp or both)

Once completed, return this form by postal mail to the address indicated in the form, by fax to (514) 766-9712, or as an attachment in an email with payment to follow.

PRINTER-FRIENDLY REGISTRATION FORM Use this form if you wish to print out the registration form, then enter your registration information by hand. Go to the print-only version of the registration form for the Benoit Barbeau Summer Hockey School and Pre-Season Camp To use this form, follow the steps described below. 1. Read the first page of the registration form for background information on the available programs and payment details. 3.
